Integrated Service Plan
The Integrated Service Plan (ISP) identifies problems to be addressed in treatment. The ISP includes measurable treatment objectives; the duration, frequency and type of services to be provided; and person(s) responsible for actions. With the client's consent, copies may be sent to all service personnel currently active with the client.
